Death Certificates
To obtain a Death Certificate for insurance policies, estate settlement, or real estate transfers, contact the Polk County Recorder's Office at (515) 286-3160.
To obtain a raised seal certified copy of a death certificate you can write to the Recorder or go to the office. They will only have the record if the person died in Polk County or if they were a Polk County resident at the time of death. They do not have death records prior to July 1, 1941.
